• the provider's cash price

The member can check any provider's biography and do a MapQuest search on any provider's address. The member selects a provider and then prints a sheet showing the product or service, the date, the price quoted, and the name and address of the provider selected. The sheet provides room to write in an appointment date and time, if applicable.
